MyChart offers patients personalized and secure on-line access to portions of their medical record. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:

When can I see my test results in MyChart?

Your test results are released to your MyChart account after your physician has reviewed them and chosen to release them.

Why are certain test results not shared electronically via MyChart?

Your provider is able to determine which types of test results are able to be accessed through MyChart. Further, tests of a very sensitive nature are not released to MyChart.

If some of my health information on MyChart is not correct, what should I do?

If you have access to MyChart, use the Ask Customer Service form located in the Message Center. You may also contact your physician's office directly.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 1-2 business days. Please do not use MyChart to send any messages requiring urgent attention.

Can I view someone else’s health record in MyChart?

Yes. A process called “proxy access” allows you to view the MyChart records of someone else. First, the patient must complete the proper proxy authorization form at his/her provider’s office to give another individual permission to access their medical information through MyChart. Both the patient and the proxy must be 18 years or older.

What is Proxy Access?

SLUCare offers proxy access for adults. MyChart Adult Proxy Access gives a patient representative access to manage health information for an adult patient. This is designed to ease the process of caring for another adult, such as an elderly parent, for couples who wish to share health information and assist one another in managing tasks such as requesting appointments or prescription renewals. With proxy access, caregivers have access to appointment histories, visit summaries, medication instructions and other important information. To grant proxy access a MyChart Proxy Authorization Form must be completed by the patient. These forms become part of the patient's electronic health record and must contain each patient’s or proxy’s individual information which is used for identification purposes and MyChart account set-up.

What are the Requirements for Proxy Access?

Proxies must be 18 years or older, have access to the Internet and have a valid e-mail account.

Can there be more than one designated proxy for a single patient?

Yes. A patient has the right to authorize more than one individual to have access to his/her MyChart information. A separate and applicable form must be completed for each proxy relationship. These forms become part of the patient's electronic health record and must contain each patient’s or proxy’s individual information which is used for identification purposes and MyChart account set-up.

Can a person serve as proxy for more than one patient?

Yes. An individual can serve as a proxy for more than one adult. A separate and applicable form must be completed for each proxy relationship.

Can a non-family member have access to a MyChart account?

Yes. Patients aged 18 or older can grant proxy access to non-family members.

Can I ask questions regarding another patient from my MyChart account?

A proxy can send a message on behalf of a SLUCare patient. If you are not a proxy for a SLUCare patient, please do not send messages about that person from your own MyChart account, since the information would not appear in the correct person’s electronic medical record.

Can a family member and I share one MyChart account?

No. Each patient or proxy must establish their own individual MyChart account in order to assure that information is associated with the correct account.

To have proxy access for a patient do I need to be a patient at SLUCare?

No. You do not need to be a patient at SLUCare. A non-patient proxy is a user with a MyChart account, but no patient record.

Can proxy access be revoked?

Yes. Proxy access to adults 18 and older can revoke proxy access. This can be done from within their MyChart account or by completing and returning the MyChart Proxy Access Revocation Form to SLUCare Medical Records.

I forgot my password. What should I do?

You may click the "Forgot password" link on the sign-in page to reset your password online. You may also contact the MyChart Team at 314-977-4440.

Can you send me a new access code as I have lost it, let it expire or did not receive it?

Contact the MyChart Team at 314-977-4440 and after we verify your information, a new code will be sent to you via e-mail or U.S. Postal Mail.

Log into MyChart and from the left menu, go to the Profile section and select the appropriate option.

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID's, and passwords. Each person controls their password, and the account cannot be accessed without that password. MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

I was automatically logged out of MyChart, what happened?

We aim to protect the privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

What do I need to use MyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer), and a valid e-mail address.

My access code does not work, what should I do?

For your security, your access code expires after 30 days and is no longer valid after the first time you use it. If you still have problems, please call the MyChart Team at 314-977-4440.

Is my access code my user ID?

No, your access code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 30 days). When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
